The provider must have all forms signed on or before the first day of care. Provider must also have a copy of your child(ren)'s immunization records. Occasionally parents will be given a packet to update all information. Click here to download application. RATES Please pay by check, money order, cashier's check, on-line payment, or cash. Tuition is to be paid weekly or Bi-Weekly. We accept Government and Subsidized Programs. Should your check be returned, an additional $25.00 bank charge will be due. |
